You can receive a Mythic+ Keystone each week, from the following sources:

If you want to make a group and don't have a keystone, you can make a m0 run and once you finish you will get a +2 key. Complete a Mythic dungeon without a key and you will be given a key, from there, you can form a group to complete it and you will get progressively better loot the higher the key gets.
For Keystone Master you should hit the 2000 mark after you complete all season three dungeons on both Fortified and Tyrannical affixes at key levels 13 and above. Keystone Hero will take a bit more time and effort because you will have to make a big push toward that 2500 mark.
